ADULT DOSE:

Bone and joint infections-Intramuscular or intravenous, 1.5 grams (base) every eight hours . Gonococcal infections, disseminated or Pneumonia, uncomplicated or Skin and soft tissue infections or Urinary tract infections, uncomplicated-Intramuscular or intravenous, 750 mg (base) every eight hours . Gonorrhea, uncomplicated-Intramuscular, 1.5 grams (base) as a single dose at two different sites, in combination with 1 gram of oral probenecid . Meningitis, bacterial-Intravenous, up to 3 grams (base) every eight hours . Perioperative prophylaxis-Open heart surgery patients: Intravenous, 1.5 grams (base) at the induction of anesthesia, then 1.5 grams every twelve hours, for a total of 6 grams . Other surgery patients: Intravenous, 1.5 grams (base) one-half to one hour prior to the start of surgery, then 750 mg (intramuscularly or intravenously) every eight hours thereafter . For all other infections-Severe or complicated: Intramuscular or intravenous, 1.5 grams (base) every eight hours . Life-threatening: Intramuscular or intravenous, 1.5 grams (base) every six hours . Note: Adults with renal function impairment may require a reduction in dose . See Cefuroxime Sodium Injection USP .

 

MAXIMUM ADULT DOSE:

 

PEDIATRIC DOSE:

Bone and joint infections-Intramuscular or intravenous, 50 mg (base) per kg of body weight, up to 1.5 grams, every eight hours . Meningitis, bacterial-Infants and children 1 month of age and older: Intravenous, 50 to 80 mg (base) per kg of body weight every six to eight hours . Neonates up to 4 weeks of age: Intravenous, 33.3 to 50 mg (base) per kg of body weight every eight to twelve hours . For all other infections-Infants and children 1 month of age and older: Intramuscular or intravenous, 12.5 to 33.3 mg (base) per kg of body weight every six to eight hours . Neonates up to 4 weeks of age: Intravenous, 10 to 50 mg (base) per kg of body weight every eight to twelve hours . Note: Pediatric patients with renal function impairment may require a reduction in the frequency of dosing consistent with adult dosing recommendations in renal impairment . MAXIMUM PEDIATRIC DOSE:

Up to the maximal adult dosage for the indication .

 

ELDERLY DOSE:

Bone and joint infections-Intramuscular or intravenous, 1.5 grams (base) every eight hours . Gonococcal infections, disseminated or Pneumonia, uncomplicated or Skin and soft tissue infections or Urinary tract infections, uncomplicated-Intramuscular or intravenous, 750 mg (base) every eight hours .
