ADULT DOSE:

Pneumonia, lobar or
Prostatitis or
Urinary tract infections, serious -Oral, 500 mg every six hours; or 1 gram every twelve hours .
Respiratory tract infections, other than lobar pneumonia or
Skin and soft tissue infections -Oral, 250 mg every six hours; or 500 mg every twelve hours .
Urinary tract infections, uncomplicated -Oral, 500 mg every twelve hours .
For all other infections, severe or chronic -Oral, up to 1 gram every six hours .
Note: Adults with impaired renal function may require a reduction in dose as follows :
Creatinine clearance (mL/min)/(mL/sec) Dose
> 20/0.33 500 mg every 6 hours
5-20/0.08-0.33 250 mg every 6 hours
< 5/0.08 250 mg every 12 hours
