PEDIATRIC DOSE:

Urinary tract infections-Infants and children 1 month of age and older: Oral, 6.7 to 13.4 mg per kg of body weight every eight hours .
Infants up to 1 month of age: Safety and efficacy have not been established .
Otitis media or

Tonsillitis-Infants and children 1 month of age and older: Oral, 6.7 to 13.4 mg per kg of body weight every eight hours; or 10 to 20 mg per kg of body weight every twelve hours .

MAXIMUM PEDIATRIC
DOSE:

Doses of up to 60 mg per kg of body weight per day have been used. However, in older children, the maximum dose should not exceed 1.5 grams per day.
