ADULT DOSE:
|
|
Bronchitis, bacterial exacerbations due to Haemophilus influenzae-
Oral, 500 mg every twelve hours for seven to fourteen days .
Bronchitis, bacterial exacerbations due to other organisms; or
Pneumonia, chlamydial, mycoplasmal, or streptococcal; or
Skin and soft tissue infections, uncomplicated-
Oral, 250 mg every twelve hours for seven to fourteen days .
Mycobacterium avium complex disease, disseminated (prophylaxis) -
Oral, 500 mg two times a day .
Mycobacterium avium complex disease, disseminated (treatment)-
Oral, 500 mg two times a day in combination with other antimycobacterial agents; clarithromycin therapy should continue for life if clinical and mycobacterial improvements are observed .
Pharyngitis, streptococcal or
Tonsillitis-
Oral, 250 mg every twelve hours for ten days .
Sinusitis, acute maxillary-
Oral, 500 mg every twelve hours for fourteen days .
Ulcer, duodenal, Helicobacter pylori -associated (treatment adjunct with amoxicillin and lansoprazole) -
Oral, 500 mg in combination with amoxicillin 1000 mg and lansoprazole 30 mg every twelve hours for fourteen days .
Ulcer, duodenal, H. pylori -associated (treatment adjunct with omeprazole)-
Oral, 500 mg three times a day in combination with omeprazole 40 mg once a day for fourteen days, followed by omeprazole alone 20 mg once a day for fourteen days .
Ulcer, duodenal, H. pylori -associated (treatment adjunct with ranitidine bismuth citrate) -
Oral, 500 mg three times a day in combination with ranitidine bismuth citrate 400 mg two times a day for fourteen days, followed by ranitidine bismuth citrate alone 400 mg two times a day for fourteen days .
Note: Combination therapy with clarithromycin and ranitidine bismuth citrate is not recommended for patients with a creatinine clearance of less than 25 mL per minute (0.41 mL per second) .
Note: The dose of clarithromycin should be adjusted in patients with severe renal function impairment (creatinine clearance [CrCl] < 30 mL/min [0.5 mL/sec]). The following dosing guidelines are suggested :
Dose for CrCl of > 30 mL/min (0.5 mL/sec) Adjusted dose for CrCl of < 30 mL/min
(0.5 mL/sec)
500 mg two times a day 500 mg loading dose, then 250 mg two times a
