ADULT DOSE:

[Endocarditis, prophylaxis] -Intramuscular or intravenous, 1 gram (base) one-half hour prior to the start of surgery . Perioperative prophylaxis-Intramuscular or intravenous, 1 gram (base) one-half to one hour prior to the start of surgery; 500 mg to 1 gram during surgery; and 500 mg to 1 gram every six to eight hours following surgery for up to twenty-four hours . Pneumonia, pneumococcal-Intramuscular or intravenous, 500 mg (base) every twelve hours . Urinary tract infections, acute, uncomplicated -Intramuscular or intravenous, 1 gram (base) every twelve hours . For all other infections-Mild: Intramuscular or intravenous, 250 to 500 mg (base) every eight hours . Moderate to severe: Intramuscular or intravenous, 500 mg to 1 gram (base) every six to eight hours . Severe to life-threatening: Intramuscular or intravenous, 1 to 1.5 grams (base) every six hours . Note: Adults with renal function impairment may require a reduction in dose . See Cefazolin Injection USP .

 

MAXIMUM ADULT DOSE:

6 grams (base) per day; however, doses of up to 12 grams per day have been used in rare instances .

 

PEDIATRIC DOSE:

[Endocarditis, prophylaxis] -Intramuscular or intravenous, 25 mg (base) per kg of body weight one-half hour prior to the start of surgery . For all other infections-Infants and children 1 month of age and older: Intramuscular or intravenous, 6.25 to 25 mg (base) per kg of body weight every six hours; or 8.3 to 33.3 mg per kg of body weight every eight hours . Neonates and infants up to 1 month of age: Intravenous infusion, 20 mg (base) per kg of body weight every eight to twelve hours . Note: Children with renal function impairment may require a reduction in dose . See Cefazolin Injection USP . 

 

MAXIMUM PEDIATRIC DOSE:

1 gram (base) for prophylaxis of bacterial endocarditis .

 

ELDERLY DOSE:

[Endocarditis, prophylaxis] -Intravenous infusion, 1 gram (base) one-half hour prior to the start of surgery . Perioperative prophylaxis-Intravenous infusion, 1 gram (base) one-half to one hour prior to the start of surgery; 500 mg to 1 gram during surgery; and 500 mg to 1 gram every six to eight hours following surgery for up to twenty-four hours .
