ADULT DOSE:

Dental anesthesia (for infiltration or nerve block)-20 to 100 mg (1 to 5 mL) of lidocaine hydrochloride as a 2% solution with epinephrine 1:100,000 or 1:50,000 . Other indications-See Lidocaine Hydrochloride Injection USP. Administration of epinephrine concurrently with the local anesthetic may permit use of doses somewhat larger than those listed.

MAXIMUM ADULT DOSE:

Dental anesthesia-7 mg per kg of body weight or 500 mg of lidocaine hydrochloride . Note: A lower limit may apply for poor patient condition, or for some sites of administration. A higher limit may sometimes apply with the use of adjunct drugs. The utility of adjunct drugs to protect the patient from side effects varies depending on the drug and the side effect. For example, the use of benzodiazepines may help prevent seizures during the period of peak absorption of the anesthetic, but will not protect the patient from its cardiovascular toxicity . Other indications- 7 mg of lidocaine hydrochloride per kg of body weight but not exceeding 500 mg as a single dose .

PEDIATRIC DOSE:

Dental anesthesia-20 to 30 mg (1 to 1.5 mL) of lidocaine hydrochloride as a 2% solution with epinephrine 1:100,000. Local infiltration or Nerve block-Up to 7 mg per kg of body weight, as a 0.25 to 1% solution . Caudal epidural anesthesia-Up to 7 mg per kg of body weight, as a 0.5 to 1% solution.
