ADULT DOSE:
|
|
Antihypertensive-Initial: Oral, 25 mg two or three times a day, the dosage being increased if necessary after one or two weeks to 50 mg two or three times a day.
Left ventricular dysfunction following myocardial infarction-Initial: Oral, a single dose of 6.25 mg. Then 12.5 mg three times a day, gradually increased to 25 mg three times a day over several days.
Maintenance: Oral, 50 mg three times a day.
Note: Captopril therapy may be initiated as early as three days following a myocardial infarction.
Diabetic nephropathy-Oral, 25 mg three times a day.
Vasodilator, congestive heart failure -Initial: Oral, 25 mg two or three times a day, the dosage being increased gradually as necessary on a daily basis up to 50 mg two or three times a day. If further increases in dosage are needed, it is recommended that they be made after an interval of two weeks so that the full effects of captopril will be apparent.
Maintenance: Oral, 50 to 100 mg two or three times a day.
Note: An initial dose of 6.25 to 12.5 mg two or three times a day should be used in patients who are sodium- and water-depleted as a result of diuretic therapy, in patients continuing to receive diuretic therapy, or in patients with renal function impairment. Such patients should be kept under medical supervision for one hour after this initial dose, to watch for excessive hypotension .
Dosage increases in patients with significant renal function impairment should proceed slowly (one- to two-week intervals), and smaller increments should be used. |

MAXIMUM ADULT DOSE:
|
|
150 mg per day for hypertension and left ventricular dysfunction, post-myocardial infarction.
450 mg per day for congestive heart failure.

PEDIATRIC DOSE:
|
|
Newborns- Initial: Oral, 10 mcg (0.01 mg) per kg of body weight two or three times a day, the dosage being adjusted as needed and tolerated.
Children- Initial: Oral, 300 mcg (0.3 mg) per kg of body weight three times a day, the dosage being increased if necessary in increments of 300 mcg (0.3 mg) per kg of body weight, at intervals of eight to twenty-four hours to the minimum effective dose.
Note:
An initial dose of 150 mcg (0.15 mg) per kg of body weight three times a day should be used in patients who are sodium- and water-depleted as a result of diuretic therapy, in patients continuing to receive diuretic therapy, and in patients with renal function impairment. |
